Maximizing Your Impact with Analytics Our analytics suite tracks trends across user, site, and program interactions. These insights are designed to help you refine staff adoption plans and community engagement strategies.

To ensure your program is meeting its goals, we’ll work with you to define specific Key Performance Indicators (KPIs). These measures are based on the workflows we established in previous phases, allowing our team to guide you toward the exact dashboards and data insights you need to measure success.

 

Defining Your Roadmap for Success

In this phase, we move from big ideas to measurable impact. Together, we will:

 

 

 

  • Establish Your North Star: We’ll define the specific goals and KPIs (Key Performance Indicators) that matter most to you. This ensures we can celebrate your wins and identify exactly where there’s room to grow.

  • Harness Data-Driven Insights: By turning data into action, you’ll be able to make informed decisions that directly improve how you address social needs within your community.

  • Digitize & Streamline: We’ll review your current processes and transition them into digital workflows. Our goal is to replace manual effort with efficient reporting, giving your team more time to focus on what they do best—helping people.

  • Understand organizational goals and needs as it relates to SDOH metrics required for various initiatives, such as NCQA Health Plus rating, Medicare Star ratings, and more

  • Ensure needs are clearly articulated with the program manager and operational lead(s) to support scoping of analytics in the project

Analytics Team Member

  • Participate in scoping discussions alongside program manager and operational lead(s)

  • Ensure required data elements are clearly articulated for the project

  • Aid with any internal development teams may have to pull needed data and analytics from your Findhelp platform

  • Assist with any additional analytics setup, such as a SQL database connection

Program Manager

  • Clearly articulate data requirements and analytics priorities for the organization

  • Ensure understanding of where data elements live, and who needs access to which dashboards

  • Create and execute on analytics strategy, including training needed staff, alongside Findhelp CSM

Operational Lead

  • Assist in user scoping for various Findhelp dashboards

  • Ensure understanding of where data elements live, and who needs access to which dashboards

  • Assist with ongoing execution and data monitoring with staff and teams

  • Review Your Evaluation Strategy: Take a look at the proposed strategy and the recommended KPIs/MOS tab within your project plan.

Having these reviewed in advance will allow us to jump straight into the details and finalize your roadmap during the call!

  • Technical Foundations: If your needs require a deeper dive, we’ll help set up and map your data structure for more advanced reporting (SQL).

What We’ll Need From You (Customer)

  • Your Reporting Needs: A detailed breakdown of the specific goals or social care requirements (SDOH) your organization is aiming to meet.

  • Your Vision: Collaboration on your analytics strategy to ensure our training matches your team’s specific skill sets and goals.

  • Resource Planning: Working with your CSM to determine if your data needs require additional technical add-ons.

  • The Long-Term View: A discussion on your "big picture" strategy to ensure you’re seeing a strong return on your investment.
