Scope + Design: Community Partnerships and Referral Pathways

Now that we’ve mapped out your internal workflows, we’ll focus on the heart of your program: Community Partnerships and Referral Pathways. We intentionally build on our previous conversations to ensure that your internal processes align perfectly with the external community.

In this phase, we’ll explore Findhelp’s network approach and curation pathways to find the partnership model that best fits your goals. Together, we will design a community investment strategy that ensures your team can connect people to care quickly, reliably, and with confidence.

Our Key Objectives

  • Design Your Network Strategy: We’ll build a tiered partnership approach that directly supports your team’s workflows, ensuring you are connected to the right community resources at the right time.

  • Define Your Roadmap for Success: We’ll identify the specific next steps needed to reach your platform goals and achieve the long-term outcomes you’ve envisioned for your community.
