Goals allow you to set important milestones related to your seeker's care. You can either select from a list of categories or create a custom goal using the description field. Goals help you track progress and organize your seeker's care plan. You can:
- Add notes
- Update the status
- Attach referrals
Finally, all goals are included when you download a seeker's profile as a PDF.
Adding a Goal
Step 1: In the Seeker Profile, navigate to the goals section and click "Add Goal".
Step 2: In the Add Goal window you can select from the drop-down menu what type of goal you are making. You may also enter in a Goal Description before clicking Save.
After clicking Save you can see your new goal in the Goals box.
When you submit a social needs assessment for a seeker, the system automatically creates goals that are directly related to their answers. This helps streamline the process of connecting seekers with the support they need.
Tie a Referral to a Goal You Have Made
To link a referral to a specific goal, click the “Add/Remove Goals” button inside the referral, then select the goal you want to connect it to.
When you add a referral to a goal, the referral then moves into the goal section under the selected goal collapsable menu.
Tracking the Status of the Goal
You can manage and track the progress of addressing goals by updating the adding a note and updating the status.
Add a Note to a Goal
Add a note to your goal to keep track of your progress.
1. In the Goals section, click the arrow next to the specific goal you want to add a note too.
2. Click the Add Note button.
Updating the Status of the Goal
You can view the current status of a goal, directly under the name of the goal:
To update the status of a goal:
1. In the Goals section, click the arrow next to the specific goal.
2. Select the appropriate status from the dropdown menu located to the left of the Goal box. The status will automatically update.
You can easily manage your seeker's progress by adding notes and updating the status of their goals. This helps you track their journey and ensure their needs are being met.
