The Inbound Referral Dashboard

The Inbound Referrals Dashboard is where you'll find all the referrals that have been approved by care management organizations and sent to your program. It's designed to give you everything you need to follow up with people seeking services.

You can get there through your referral notification, or by going to nwfhealthconnects, signing in, and clicking on My Program Tools and then Inbound Referral Dashboard.

What You'll Find on Your Dashboard

Each referral includes:

  • The seeker's contact information so you can reach out and start providing your service.
  • A copy of the screener that the case manager submitted with the referral.
  • A way to update the referral status as you work with the seeker.

A Dashboard Overview

Within your dashboard you will see the following: 

     A. Seeker being referred

     B. Referral screener filled out by the case manager

     C. Referral status update drop down

     D. Service reimbursement button

New referrals are easy to find. They will have a "New" badge, a highlighted background, and bold text. This helps you quickly see which ones need your attention.

Referral Statuses

Referral Statuses are a tool that helps everyone involved in a referral stay informed. This feature allows the seeker (the person who needs help), the navigator (the person who made the referral), and the program that received the referral to collaborate on tracking its progress. There are two types of statuses: pending and closed.

Pending Referral Statuses

A 'Pending' status means the referral is still in progress. Be sure to update the status as you work on it. This helps everyone involved stay informed and can cut down on the number of follow-up questions.

Not Updated: This is the default setting all referrals come in under.

Needs Client Action: The Seeker needs to do an action before services can be provided.

Pending: This can be used by the program receiving the referral and means it is being processed.

Eligible: The Seeker is eligible for the service but has not received it yet.

 

Closed Referral Statuses

A closed status indicates that the referral process is complete. Changing a referral to any of these statuses finalizes the referral loop and provides a clear outcome.

Referred elsewhere: You have directed them to a different organization for help.

Got help: You have helped them!

Couldn’t contact: You were unable to contact the Seeker.

Not eligible: The Seeker was ineligible for the program.

No capacity: The program doesn’t have capacity to help the Seeker.

Couldn’t get help: The Seeker was unable to get help for various reasons from the program.

No longer interested: The Seeker indicated they no longer need or have an interest in the program.
