The New Mexico Findhelp and YesNMConnect State Summit, convened by Findhelp in partnership with the New Mexico Health Care Authority, brings together state leaders, agencies, health plans, providers, and community partners to advance a shared vision for a more connected system of care. YesNMConnect—New Mexico’s statewide closed-loop referral initiative led by the Health Care Authority—serves as the foundation for strengthening cross-sector collaboration and coordinated service delivery. The summit will highlight how aligned goals, integrated systems, and community partnerships improve access to services, strengthen local support networks, and elevate community-driven solutions. Together, this work reflects a statewide commitment to shared accountability, collaboration, and collective impact.

Guided by Community and Connected Across New Mexico

Health Care Authority, Deputy Secretary, Katherine Slater-Huff will share New Mexico's vision for a truly connected social care system that honors the diverse cultures, communities and traditions that define our state.  This keynote will explore the importance of strengthening coordination in social drivers of health and community-based care in New Mexico, highlighting current priorities, emerging opportunities, and the strategic direction guiding Yes NMConnect and closed loop referrals.  Attendees will gain a clear understanding of how state policy, local leadership, and cross-sector collaboration are coming together to create a more unified and responsive structure that leads to meaningful support and communities that are active partners in shaping solutions.

This session will highlight how Findhelp technology , when guided by values and state leadership, strengthens collaborations across state agencies, counties, managed care organizations, and community based partners, and supports the people that make care possible. Attendees will gain insight into how Yes NMConnect and Findhelp are building enhanced workflows, trusted networks, interoperability and purpose-built initiatives and how communities remain at the center of this work.

Connecting Public Health, Aging, and Community Support

New Mexico state agencies play a critical role in shaping how residents access health and social care.  In this session, leaders from the Aging and Long-Term Services Department (ALTSD) and the Department of Health (DOH) will share how they are integrating YES NMConnect into their programs to strengthen coordination, visibility and service delivery.

Community at the Center of Care County and Community Organization Spotlights

Local leaders from counties and community based organizations will share their journey and progress along with the future vision of their work. Panelists will discuss how cross-sector collaboration is helping to make it easier for residents to find, access and receive the support they need. In this conversation, leaders will also explore how they are leveraging the Closed Loop Referral technology to elevate visibility of local resources and drive more coordinated engagement for their counties.

Healthcare as a Community PartnerNM Managed Care Panel

Collaboration and data interoperability are essential to advancing whole-person care across New Mexico.  In this session Turquoise Care Managed Care Organizations will share how they are strengthening care coordination across the state.  Panelists will discuss how referral transparency and integrated data systems will reduce duplication and improve continuity of care.   Attendees will learn how MCOs are building system structures that align with New Mexico's priorities, leveraging the CLRS platform to support measurable impact, while maintaining strong relationships with community partners.
