Solving for Social Drivers of Health & Collaborating for Change

This session focuses on how Advocate Health has worked to to integrate social care through the successful centralization of fragmented Community Health Worker (CHW) models and the intentional alignment of community partnerships. The centralization of CHWs into a unified, enterprise-level structure allows for the standardization of processes for patient triage, referral, and tracking, especially for those with social drivers of health (SDOH). This approach is shown to improve outcomes and consistency while maintaining a relationship-centered care focus. In parallel, Advocate Health is unifying Social Health, Community Engagement, and Corporate Social Responsibility (CSR) to create a cohesive ecosystem for partnership strategy and funding. By collaborating across silos and grounding decisions in shared values and social health data, this model enables more intentional, equitable, and scalable investments, strengthening trusted community relationships and creating innovative referral pathways, as illustrated by a real-world example during a period of community need.
